Services

California Heart Connection provides the following services for families with children with congenital heart defects, and adults with congenital heart defects. All services are free (we sometimes ask for a small donation which is optional). We do ask that you fill out a membership form to participate in these services. All information is kept confidential unless you give us express permission to share it with other CA Heart Connection members for matching purposes.

Events

CA Heart Connection holds events such as our Annual Heart Picnic, fundraising events and seminars for those affected by CHDs. Our Annual Heart Picnic allows families and adults with CHDs to share information and support, and just have fun! See information on our popular Picnic and other upcoming events on our events page.

Online Support Group

We offer members the opportunity to ask questions and share information via an online support group. Please fill out a membership form FIRST and then follow these instructions to join our discussion:

1. Go to: http://groups.yahoo.com/group/caheartconnection/ and sign in using the same email address you listed on the membership form.

(PLEASE NOTE: The support group will not recognize any email address other than the one listed on your membership form.)

2. You will receive a notice when you have been added, along with a set of guidelines for the online support group. Then, you will be able to post to the group.

Matching

Upon request, we will help match members with similar conditions and circumstances. If you are interested, please contact us via email or call us at 949-653-6421.

Resources

We strive to make those affected by CHD's aware of the resources and services available to them such as free Early Intervention services to help their child achieve his or her potential. Our Resource Guide include tips, helpful products and questions to ask the doctors. Members without internet access can receive printed materials including the resources listed on this website upon request. Our Resource Guide is now available in spanish!

Free, Downloadable Handouts and Forms

California Heart Connection has created several handouts and forms to help parents and individuals keep track of important information. Forms such as "When to Call the Doctor," the "Emergency Information Sheet" and "Physical Activity Form" will help parents keep important information in writing to refer to, or share with caretakers and school officials. These forms can be downloaded for free by patients and physicians. Physicians may also customize these forms to include their contact information, and personalize them to each patient.

CA Heart Connection CarePages

CarePages are free, private, personalized Web pages available to CA Heart Connection members to help you stay in touch before, during and after a hospital stay, or any time care giving is needed. The service allows you control over communication and provides an easy way for friends and loved ones to respond with messages of support.

CarePages provide a convenient way to keep loved ones updated by sharing photos, providing contact information and visiting hours and relaying information when the time is right for you.
