Articles, handouts and forms are provided as a courtesy to help improve communication between physicians and patients to increase patient understanding of the many issues involved and to improve outcomes of those with congenital heart defects.
Please feel free to download the following forms and use for patient education. Fill in the desired information and print for patients.
To Personalize Forms: A space has been created in the upper right hand corner of each form for physicians to include their contact information. Simply input your name, address, phone, email and other information.
